Family Support Coordinator
What is a Family Support Coordinator?
The Family Support Coordinator is a support person to ICW resident members and ICW families. The Family Support Coordinator works with each ICW family to create and maintain a care plan for their ICW resident member. The Family Support Coordinator monitors the implementation of the care plan before, during and after the family support life change. The Family Support Coordinator also educates residents on available services, develops contacts with service providers and agencies for resident member referrals and monitors provision of services.
